Registering with the practice
We are using a new online service called Register with a GP surgery that makes it easy to register with this GP surgery.
Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.
The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Catchment area
The practice list is currently open and we are happy to accept new patients who live within our boundary area (This includes Hartlepool and also the surrounding villages of Hart, Elwick, Dalton Piercy, Greatham and the Headland).
Before filling out a registration form, check you are in our catchment area using the map and postcode checker opposite.

- please visit reception and request a GMS1 registration form and New Patient Pack
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
Practice Boundary
Accountable 'Usual' GP
All patients registered with a GP practice are allocated a “Usual GP”. This is the named accountable general practitioner who is responsible for a patients overall care at the practice.
Patients registered at Gladstone House Surgery are allocated a 'Usual GP' based on Surname.

- You may wonder why your allocated GP is not necessarily the one you see most regularly. Please be assured that you can still access all our medical team in the same way, and you will not see a change in the way your care is delivered.
